Scantron form processing

While most courses are evaluated online, some courses may still be evaluated in-class. Students fill out Scantron forms, or bubble sheets, with their answers. Comments may be written on the sheets as well. It is up to the departments to purchase the blank forms for student use. Once completed, Scantron forms can be scanned by ITS, in the NOC (Network Operations Center) in the Hill Center.
  1. Create a header sheet for each section
    • The Microcomputer Labs can provide what few blank header sheets you might need for in-class evaluations.
      Call 2-6950 or email ted.payne@vanderbilt.edu
    • Bubble in as follows:
      Col 1 Asterisk (*)
      Col 2-5 AREA (HIST, MATH, FA)[left-justified]
      Col 6 blank
      Col 7-10 COURSE (100, 101A, 115W)[left-justified]
      Col 11 blank
      Col 12-13 SECTION (01, 02, 03)
      Col 14 blank
      Col 15-18 #Students ENROLLED (9, 22, 132)[right-justified]
      Col 19 blank
      Col 20-22 100 (just do it)
      Col 23 blank
      Col 24-47 Name of instructor (last, first middle)[left-justified]
  2. Place the header sheet on top of each section's stack of forms
  3. Download and fill out this Scanning Request Form from ITS
  4. Take them to the Network Operations Center, located in 142 Hill Center on the Peabody Campus

  5. Once forms are scanned and requested reports generated, you may pick up your forms and reports from the same location.
